Dass, according to Bhai Randhir Singh ji reaching a thoughtless state is no big deal but a low stage, a true gursikh is an aath pehari, he meditates on gurmantar with each breath 24/7. My understanding is that if you commit a bujjer kurehit even once after you first take amrit you cannot do seva in the punj, even if you retake amrit. If you commit an anti gurmat offence you cannot do seva in that present condition. There are many sants today and respectable sant marapurahs of the past who have done huge kmayee and have learnt jugtis and passed on these jugtis. When researching their experiences, I just think vah! how great are gurmukhs like bhai sahib Randhir Singh Jee whos avastha is way beyond these sants. With gurmukhi script there is a continuous line on top of letters like "----------" what ive noticed is that some words clearly have extra "--" between 2 letters in that particular word. Correct me if i'm wrong but the only reason peshi for surgery is required is if the surgeon shaves the hair. For health reasons hair must be shaved in the area of cutting through skin. For all other operations peshi wont be required. I've heard some gursikhs in past have made sure surgeon doesn't shave off any hair in the region of cutting. Important thing to note is that it doesn't really matter if kes or keski is the kakar. What is a must is that both singhs and bibis keep dastar rehit as outlined in your last point.
